Bowel and Bladder Service
Referral Forms for the Bowel and Bladder Service are available:
- On the EMIS Centre of Excellence website
- In the Systm1 Organisation Group folder
The referral should be sent to Optum via the NHS e-Referral Service through a urology slot.
The Bowel and Bladder Service referral form now includes Lower Urinary Tract Symptoms (LUTS). Please note that this is for guidance only and should not replace clinical judgement.
The Bowel and Bladder service offers individuals with bladder and bowel problems assessment, treatment, advice and support in the community. The service covers (but is not limited to) the following issues:
- Erectile dysfunction (ED)
- Faecal incontinence
- Urinary incontinence
- Urinary frequency
- Intermittent catheterisation
- Lower Urinary Tract Symptoms (LUTS)
The Bowel and Bladder Service referral form now includes LUTS.

A Bladder Diary and Prostate Symptom Score Tool are available in the table below. These are intended to support diagnosis, but a prostate symptom score is not required for a referral to be accepted.
